Telmisartan: The 'patron saint' of Hypertension Management

 Introduction

Hypertension, as a global chronic disease, has become an important killer that threatens human health. In the treatment of hypertension, angiotensin II receptor antagonists (ARBs) have attracted much attention due to their unique antihypertensive mechanism and few side effects. As a member of the ARBs family, it has become a star drug in the field of hypertension treatment due to its highly effective antihypertensive effect and good tolerance.1

Pharmacological mechanism

Telmisartan is an orally effective specific angiotensin II receptor (AT1 type) antagonist. By binding to the AT1 receptor, Telmisartan blocks angiotensin II's vasoconstriction, water, and sodium retention, and promotes cell proliferation, thereby reducing blood pressure. In addition, it inhibits the overactivation of the renin-angiotensin-aldosterone system (RAAS), reducing the burden on the cardiovascular system and improving cardiac function.2

Clinical application

At the heart of Telmisartan's efficacy lies its unique mechanism of action.  As a member of the angiotensin II receptor blockers (ARBs) class, it exerts its effects by selectively antagonizing the angiotensin II type 1 (AT1) receptor, thereby disrupting the vasoconstrictive and aldosterone-secreting actions of angiotensin II. The clinical utility of Telmisartan spans a broad spectrum of cardiovascular disorders, ranging from essential hypertension to heart failure and diabetic nephropathy.  Numerous landmark trials, including ONTARGET and TRANSCEND, have demonstrated its non-inferiority to other antihypertensive agents and its superiority in certain patient populations, heralding its widespread adoption in clinical practice.  Moreover, emerging evidence suggests promising roles for it in secondary prevention, atherosclerosis regression, and neuroprotection, underscoring its versatility beyond blood pressure control.4

Adverse reactions and drug interactions

Although it is well tolerated, some adverse reactions may still occur during use, such as dizziness, headache, nausea, etc. Most of these adverse reactions are mild and transient and usually do not require special treatment. However, a very small number of patients may have serious allergic reactions or angioedema and other serious adverse reactions, requiring prompt medical treatment.

In terms of drug interactions, Telmisartan, when combined with digoxin, warfarin, and other drugs, may change the plasma concentration of these drugs, thus affecting their efficacy. Therefore, when using Telmisartan, its interaction with other drugs should be fully considered to avoid unnecessary risks.5
